    Make sure that the sheet is double sided - one green side and one red side. Check if you have the right origami paper.

    Fold one corner diagonally to the another. Crease the fold.

    Not really, but it might work better if you do. If you don't have any origami paper, you can usually just use printer paper cut into a square.

    No, you cannot use cardboard because that will not fold. If it rips a bit, then it should be fine, but if it's larger than about 1/8 of the length of the paper or on any major creases, then you should use another piece of paper.
