In Bleach, Soul Society represents this sort of Heaven where souls are sent by Shinigamis (mostly from Earth?). However, once in Soul Society, is a soul immortal? Can they die? And if so, what happens? Is one sent to... Hueco Mundo?

The Human World and Soul Society are parallel to each other and are two sides of the same coin. Families separated by death are rarely reunited in the Soul Society unless they arrive in Soul Society together. People live nestled together like a family of strangers. One never gets hungry (if she or he doesn't have spiritual powers) and aging is slowed to a great extent, with lifespans of 2000 or more years not being unheard of, though such ages are limited to Shinigami or other Soul Society dwellers with high spiritual power.
and to answer your question,
A soul that dies in Soul Society is reincarnated on Earth as a new Human with no past memories.
Edit: However, given the huge difference in lifespan, Soul Society should accumulate souls and the Human World should run out of souls...
souls can "escape" this cycle by turning into hollows and entering Hueco Mundo (or god knows which new worlds Bleach will come up with). Souls can be exterminated permanently by Quincy:
Quincy completely extinguish Hollows. Thus the souls that come to the Human World do not return to Soul Society and therefore the souls would increase only on the side of the Human World. Meaning, the Human World becomes heavy in souls causing Soul Society to spill over into it; mixing life and death. The ultimate result would be the end of the world.

Perhaps you were wondering if "death" for spirits is the same if they are not inside Soul Society, which is the case for Ichigo when he became a Reaper. The answer then depends on what "control procedures" Soul Society has for reincarnation of spirits. However, if Ichigo was eaten by a hollow, his soul would stay inside the hollow until it was purified. – Arturia Pendragon Dec 15 '12 at 05:55

@MadaraUchiha: Hmm, if that were the case, wouldn't a hollow be unable to absorb the souls it eats, and wouldn't there be two hollows after one hollow eats a soul? I don't remember that happening. I based my "soul-absorbing" explanation on [Menos](http://bleach.wikia.com/wiki/Hollow#Menos_Classifications) which are conglomerates of Hollows, but we can also recall the [Grand Fisher](http://bleach.wikia.com/wiki/Grand_Fisher) using eaten souls as puppets. – Arturia Pendragon Dec 15 '12 at 07:47

@Robert: sorry, that part wasn't well-thought-out. I was pointing out a great imbalance between Soul Society and the Human World - namely that Soul Society must accumulate souls, depleting souls from the Human World. perhaps there is a large backlog of souls just waiting in the ether until they have an opportunity to be born into a human body; the surplus could explain why there's no need to have parity between SS 'deaths' and human births. Alternately, if there's a shortage of souls, perhaps the waiting bodies turn out as miscarriages or stillborn or something? – Meir Illumination Aug 15 '15 at 23:35

Comment on author's edit: If we ignore factors such as hollows and Hueco Mundo, and focus only on Earth and Soul Society, then difference in the amount of time souls spend in the two worlds **does not** cause accumulation of souls.
